The PHI HAA Risk page "Sign form" button should be changed to a LARGE, OBVIOUS button that changes color/shape/size when pushed. It should be relabeled to "LAUNCH"

The current iteration of the PHI HAA Risk page has a selectable button at the bottom labeled "Sign Form". This button should be larger and it should be more visually obvious when it has been pushed. Additionally, to encourage/take advantage of positive habit transfer it should be labeled "LAUNCH" to make it similar / consistent with the PHI legacy desktop application known as PRAW that served the same purpose for many years.
