For FV Transport app requests, include Receiving Unit and Receiving Room to fields for requestor to include.

We have a requestor who utilizes the FV Transport Request app and they are requesting to add the “Receiving Unit” and “Receiving Room” to the fields at the input of the request. Currently, they are entering it in the notes and they forget to add that information. I do not want to have to create a custom field for this to be an available area for the requestor to enter this information since the patient pickup unit and patient pickup room are available.
