The current custom form builder (used in Flight Records, Safety Reporting, etc.) is difficult to use and very inflexible. Is there another, more user-friendly "tool" that could be employed to build custom forms?

It would be good for the risk codes (1A, 2A, 3A etc.) to appear as well as the risk index name e.g. (Very low, low, medium, high, very high). At the moment only the risk index name appears, not the code identifier as well.
