I've noticed that it is sometimes a bit difficult to decide on a risk rating when filing a safety report as one should always open the tooltip to check in which area the appropriate risk will fall on the Risk Matrix. This is espesially the situation when there are more than one field of explanations under a certain area, in this case "consequences".
My recommendation is to have the Risk Matrix, including the explanations, permanently displayed when lodging a safety report. Additionally, it would be great if the risk matrix is interactive, thus when filing a report, you can just click on the required risk rating on the risk matrix model.

This option was available in our previous system, and was very useful. It helps make more accurate initial evaluation thus the accuracy of our stats on the capability to reducing risk. We could also use the " ? " or something else to make the grid appear when needed.
