Form Module Improvements - Locations Register integration (Bases/Locations + properties)

Problem: Only Base names available; need richer location data.

Actionables:

• Add data source picker: Locations Register.

• Allow mapping of fields (name, phone, email, coordinates).

• Support search and filter by region/base type.

Success criteria:

• Dropdowns can list and autofill location properties; selections remain current with register updates.
